Is it possible to sleep without a pillow?

You have probably woken up at least once feeling like your neck is “stuck,” and your head feels five times heavier than usual. In such moments, the pillow is the first suspect: too firm, too high, or just completely wrong. And then a thought sneaks in: “Maybe I should ditch it altogether? Our ancestors slept on flat surfaces and were fine!”. The internet is full of advice claiming that sleeping on a flat surface is the path to eternal youth and a healthy back. But is it really that simple? Let’s figure out whether modern people actually need a pillow or if it’s just a habit we should get rid of.
Is it healthy to sleep without a pillow: do you need a pillow for sleep? | Olvi Blog

Why do you need a pillow?

First of all, the human body is not straight — it has natural curves. If you look at yourself in profile, you’ll notice that your spine is not straight like a stick. There is a natural curve in the neck area. When we lie down, a gap forms between the back of the head, the neck, and the mattress. A pillow essentially acts as a “bridge” that fills this space.
The main purpose of this accessory is to support the head so that the spine remains aligned from the crown to the tailbone. Without this support, the neck muscles cannot relax. They work all night “on overdrive,” trying to keep the head in a safe position. As a result, instead of rest, the body gets a night shift — and you wake up with a headache. That’s why at Olvi, when we talk about quality sleep, we always emphasize proper anatomical support.

What happens if you sleep without a pillow?

If you decide to experiment and remove your pillow, your body will notice immediately. It’s important to understand what happens if you sleep without a pillow regularly. First, blood circulation to the brain may be affected. When the head lies below the level of the spine or at an unnatural angle, blood vessels can become compressed. This can lead to fatigue, brain fog, and a feeling of exhaustion.
Second, your face may suffer. If you sleep on your side without support, your face is literally pressed into the mattress. This can cause puffiness and early wrinkles, often called “sleep lines” by cosmetologists. And more simply — drooling and snoring. Without proper support, the jaw can shift backward, partially blocking the airway, which definitely doesn’t add romance to your sleep. When asked whether it’s better to sleep with or without a pillow, orthopedic doctors usually give a clear answer: with support. However, as with any rule, there are exceptions.

Benefits of sleeping without a pillow

At the same time, there are some rare cases where sleeping without a pillow can be beneficial. The only people who may truly benefit are those who sleep exclusively on their stomach. In this position, even a low pillow forces the neck into an unnatural backward bend, which can be harmful. Without a pillow, the spine stays more aligned.
Also, newborns are often advised to sleep without a pillow since their bones are still soft and spinal curves are just forming. However, even in this case, it is better to use a special orthopedic pillow for children that gently supports the head without lifting it too much.

Risks of sleeping without a pillow

For most people, the answer to why sleeping without a pillow is not recommended lies in the risk of developing osteochondrosis. When the neck remains under tension all night, the intervertebral discs wear out faster. If you already have issues with blood pressure or frequent headaches, sleeping on a flat surface may worsen the condition.
By the way, people with gastrointestinal issues are strongly advised not to sleep flat. In such cases, a special anti-reflux pillow is often used to keep the upper body elevated at the right angle, preventing heartburn. Without it, nighttime rest can turn into discomfort with burning sensations in the chest.

Who can sleep without a pillow?

So, should you personally sleep without a pillow? If you have broad shoulders and prefer sleeping on your side — definitely not. The distance between your ear and shoulder is significant, and without a pillow, your head will simply drop downward. If you are petite and sleep on your back on a very firm mattress, the discomfort may be less, but the benefits are still questionable.
Often, the desire to get rid of a pillow arises when the old one has worn out: the filling has flattened, it has become too thin, or turned lumpy. There’s no need to suffer. Just search something like “best pillows for sleeping” and choose options that match your habits. Many people prefer memory foam pillows — and for good reason. This “space-age” material adapts to your body’s contours, giving that weightless feeling people often seek when trying to sleep without a pillow.

How to choose a pillow?

So, the idea that sleeping without a pillow improves health ultimately leads us to the need to choose the right one instead. A good pillow should not feel like a huge cloud you sink into — it should provide support.
When choosing, consider your preferred sleeping position. For side sleepers, a higher and firmer pillow is better. For back sleepers, a lower one with neck support is ideal. Memory foam options gently “hug” your neck without creating counter pressure. If you travel often, don’t forget that your neck needs support even on a plane — otherwise, your vacation may start with a visit to a massage therapist.
We also recommend reading our guide “How to choose an orthopedic pillow” to avoid mistakes with height and firmness. Remember: investing in quality sleep is investing in your mood and productivity for the entire next day. If you decide to buy an orthopedic pillow, trust your ощущения — it should feel comfortable from the very first second.
Ultimately, sleep is a deeply personal process. But nature has already designed our spine with curves. Our job is not to fight anatomy, but to support it. Sleep well, take care of your neck, and may every morning truly be a good one!
